    Description

For auction is an original framed Volunteer Enlistment form for service during the Civil War by a physician from Augusta Maine. The form was completed on December 7, 1863 and was for a term of three years. The volunteer's name was Daniel Forbes. I found that Dr Forbes died in September 1865 in Florida while serving as the head of a hospital for those injured  during the war. The enlistment form is in excellent condition with only one 1/2" tear on one of the folds. Some of the ink is faded so it takes some concentration to decipher what has been written on the form. Actually. it looks like two different pens were used on the form - one that has stood the test of time better than the other.
